The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

PLEASE NOTE: Minors must be accompanied by their parent or guardian for both consultation and surgical appointments.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please contact our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

  
Dental Insurance

We currently offer “assignment of benefits” for those with dental insurance.  In other words we bill the insurance company first, with the remainder being billed to you after they have paid.

For patients without dental insurance or for services not typically covered by dental insurance (e.g. dental implants) payment is due when the service is provided.

If you have dental insurance benefits, please bring your insurance information so that we can submit to your insurance provider.  We will work with you to maximize your insurance benefits and routinely send a pre-authorization for you so that you know what your out of pocket expenses will be.

If you would like us to help you to understand your coverage (particularly the pre-authorization you receive from your insurance provider) please let us know.

Alberta Health Care

Some oral and maxillofacial surgical procedures have been deemed medically necessary and may be covered by Alberta Health Care. Please bring your current Alberta Health Care number with you to your consultation appointment. As an accredited non-hospital surgical facility, we are often able to perform these procedures in the office rather than the hospital.
